Speech-Language Pathologist (SLP) Home Health

Serving Oakland County & Wayne County

Your Responsibilities

In this role you will:

  • Complete speech language cognitive-communication and swallowing assessments

  • Develop individualized care plans tailored to each patients goals

  • Provide therapy services based on physician orders and best-practice guidelines

  • Coach patients and caregivers on exercises techniques and home programs

  • Document your visits clearly and promptly

  • Partner with nurses therapists and physicians to ensure seamless care

  • Promote safety and help reduce avoidable hospitalizations

What You Bring to the Team

  • Masters degree in Speech-Language Pathology

  • Active SLP license in Michigan (or Educational Limited License with required supervision)

  • At least one year of SLP experience; home health or hospice is a plus

  • Strong communication and organization skills

  • A patient-first compassionate approach to care

  • Reliable transportation and valid drivers license
