Position Summary


$23.44 - $31.50 per hour*

The Lincoln County Winter Shelter is open November 1st through April 30th.

Lincoln County is once again gearing up for the winter months to provide shelter and safety for some of our most vulnerable citizens. We are looking to increase the awareness of resources and assistance in connecting to those resources by adding a Houseless Outreach and Resource Navigator into the shelter program. This person will assess the physical health mental health and social support needs of unhoused individuals supported by the Lincoln County Shelter Program. They will provide services to those supported in the Emergency Winter Shelters and those identified through outreach efforts providing navigational support for identified individuals (i.e. linkage monitoring advocacy documentation) as needed.

Essential Functions/Major Responsibilities

This position involves the use of County vehicles and occasional transportation of clients.


Screening and Service Planning:

  • Screen unhoused individuals for the social determinants of health using tools identified by the HHS Department.
  • Determine health and social support needs.
  • Develop plans to address resources and support needs.
  • Document findings according to procedures established by the County HHS Department.

Linkage and Monitoring:

  • Carry out the traditional functions associated with service navigation to include linkage monitoring and advocacy.
  • Provide coaching utilizing motivational interviewing strategies to encourage individuals to register for and accept available services and supports.
  • Assist individuals with Medicaid/Oregon Health Plan coverage application.
  • Link/refer individuals to needed services using electronic platforms determined by the County HHS Department.
  • Monitor track and report on the status and progress of identified individuals.
  • Respond to referrals from health care providers or community partners to support people with economic logistical and other non-clinical barriers to accessing care.

Typical Qualifications

Minimum Education Level

High School Diploma / GED


Minimum Experience Level

Two- Three years experiencein a Health and Human Services setting working with marginalized or vulnerable populations.


Certifications

Specifics: Must be able to obtain OHP Assistance Certification within six months of employment.


Drivers License

Oregon Drivers License required within 30 days of hire.


Desirable Qualifications: Bilingual and/or bicultural competencies for the Latino population are preferred.


Knowledge of:

  • Needs of underserved and vulnerable populations.
  • Health disparities and trauma informed service delivery.
  • Basic HHS programs community programs and services available.
  • Eligibility and enrollment rules and procedures and coverage options under Medicaid OHP or the Health Care Market Place.


Skill or ability to:

  • Use initiative and judgement in carrying out tasks.
  • Advocate for client needs.
  • Screen individuals for social risk factors. Provide coaching and social support by utilizing motivational strategies to encourage individuals to obtain needed physical and mental health services. Link individuals to community resources as needed.
  • Maintain client confidentiality in accordance with HIPPA and CFR-42.
  • Build rapport and relationship with client necessary to assist in removing barriers and moving client toward clients goals.
  • Convey information ideas and facts both orally and in writing to supervisors colleagues and individuals inside and outside the county using language and a format the audience will understand.
  • Utilize computers and other technology to communicate in written and verbal forms (e.g. word processing texts emails spreadsheets department-specific software).
  • Effectively relate well with others including supervisors colleagues and individuals inside and outside the county. Exhibit a professional manner in dealing with others and working to maintain constructive working relationships.
  • Take personal responsibility for the quality and timeliness of work. Show up to work on time and follow instructions policies and procedures.
