Position: Social Care Navigator

Hours: 37.5 hrs. / 52 wk.

Salary Range: $40674 - $52876

Salary Grade: 9

Summary of Responsibilities:

Under the direction of the Program Director the Social Care Navigator assists in the management coordination and delivery of activities within the Social Care Network ensuring members are connected to essential health-related social care services that support overall stability and self-sufficiency. The Social Care Navigator conducts screenings assessments referrals and follow-up activities to identify members needs related to housing transportation and nutrition and other social drivers of health.

This position plays a key role in guiding members through a holistic service process helping individuals understand their needs access supportive resources and engage in services that enhance personal social and economic well-being. The Social Care Navigator collaborates closely with internal program staff community partners and social service agencies to ensure coordinated care and timely service connections.

Additional responsibilities include providing individualized screening and navigation; offering information and referral assistance; documenting all member interactions; maintaining accurate data and reports; completing eligibility screenings using standardized tools; and monitoring service progress to promote member success and program compliance.

The Social Care Navigator also contributes to data integration by ensuring accurate entry reporting and safeguarding of member information. This position strengthens community partnerships and supports advocacy efforts that enhance access to essential services. Working collectively with the program team the SCN ensures that enrollment service engagement and successful outcomes are achieved and in alignment with the mission of the Social Care Network and Action for a Better Community.

Qualifications / Education and Experience:

Education: Associate degree in health social services or related field preferred. Equivalent work experience in a related field may be considered in lieu of degree requirements. Should be willing to attend recommended approved training which provides skills/knowledge to perform or enhance the job duties.

Experience: Minimum of 2 years related experience in a clinical non-profit or Managed Care Organization (MCO) environment preferred.

  • Knowledge of health equity social drivers of health and social care data.
  • Excellent communication and presentation skills.
  • Experience using translation services preferred.
  • Ability to build collaborative relationships across organizations and community partners.
  • Skilled in multitasking documentation and data reporting.
  • Detail-oriented with strong time management.
  • Requires a valid NYS Driver s License and access to reliable transportation.
  • Proficient computer skills and willingness to learn additional applications.
  • Demonstrated commitment to diversity equity and inclusion.

Job Duties

  • Manage incoming referrals for screening and navigation to ensure timely service connections.
  • Assign and track members connected to care teams for continuity of services.
  • Conduct outreach in alignment with required time limits and protocols.
  • Obtain and manage member consent and attestation as required.
  • Conduct HRSN screenings using the Accountable Health Communities (AHC) screening tool.
  • Assess member eligibility for enhanced HRSN services and refer to appropriate programs.
  • Maintain effective communication with internal teams community members and partner organizations.
  • Document all activities in accordance with Network Standards and Quality Program requirements.
  • Use data tools to monitor referral patterns and trends reporting to the Referral
  • Coordination Manager.
  • Adhere to all ABC and funder policies including HIPAA and confidentiality standards.
  • Demonstrate alignment with ABC s Mission Vision Strategic Plan including being collaborative innovative community focused culturally competent and inclusive.
  • Local travel may be required for meetings community events and related responsibilities.
  • Requires proficiency in use of Microsoft Office products including outlook.
  • Bi-lingual capabilities preferred: English and Spanish preferred.
  • Requires the physical health and physical capability to work in an office and classroom setting.
  • Requires a valid NYS Driver s License and access to reliable transportation.

This position involves sedentary work with occasional standing or walking. The role requires use of office equipment such as computers phones and printers. Local travel may be required for meetings community events and related responsibilities. The employee may occasionally lift and/or move up to 10 pounds (i.e. supplies/program materials/mailings etc.). Specific vision abilities required by this job include close vision distance vision peripheral vision and ability to adjust focus. While performing the duties of this job the employee is regularly required to take and provide verbal & written directives/instruction; and must be able to hear and respond satisfactorily. Some evening and weekend hours may be required at times in a variety of locations in the community.
