Job Description

Job Overview

We are seeking an experienced Masters-Level Social Worker (MSW WA LICSW/LSWAIC/LASW) for a Case Manager position in its PACE program. This is an 8-week travel contract with strong involvement in case management community-based assessments and hospital/SNF discharge planning.

  • Assignment Length: 8 weeks

  • Role: Case Manager (MSW)

  • Setting: Community clinic with moderate in-home assessments

  • Travel: Mileage reimbursement & paid work travel time (between facilities and participant homes; not personal commute)

  • First-Time Travelers: Not accepted

  • Locals: Accepted

  • Weekend/On-Call: None

  • RTO: Must be declared at submission and confirmed with hiring manager

Responsibilities
  • Complete initial and 180-day biopsychosocial assessments for assigned participants.

  • Facilitate and coordinate hospital and SNF discharges ensuring safe and appropriate transitions of care.

  • Work closely with hospital discharge planners and communicate recommendations from the Providence PACE Interdisciplinary Team (IDT).

  • Manage a caseload of up to 1:50 patient ratio providing care coordination and follow-ups.

  • Conduct moderate in-home visits within the Edmonds Auburn radius.

  • Develop implement and revise individualized plans of care based on patient needs and IDT input.

  • Provide counseling crisis intervention patient/caregiver education and community resource referrals.

  • Document assessments care plans and patient progress in Epic and maintain compliance with facility standards.

Key Requirements
  • Minimum 2 years social work/case management experience.

  • Prior travel experience required; first-time travelers not accepted.

  • Experience in community hospital settings required; LTAC and Trauma Level I preferred.

  • Strong familiarity with Medicare/Medicaid HCBS and community-based programs.

  • Must be licensed in Washington State (see below).

Qualifications

Education:

  • Masters Degree in Social Work (MSW) required.

Experience:

  • At least 2 years experience in case management or social work (hospital/community-based preferred).

  • Skilled in discharge planning interdisciplinary collaboration and patient education.

License/Certifications:

  • Required:

    • BLS (Basic Life Support)

    • MSW

    • One Washington State license: LICSW LSWAIC or LASW

  • Preferred: LMSW or additional state social work credentials.

Skills:

  • Case Management/Utilization Review: admissions discharge planning appeals/denials continued stay reviews prior authorizations.

  • Protective Services: APS guardianship POA emergency detention.

  • Assessment Tools: biopsychosocial Geriatric Depression Scale Clock Drawing Dementia Screen.

  • Documentation/Communication: Epic plan of care documentation discharge summaries.

  • Counseling/Therapy: crisis intervention short-term therapy contingency planning.

  • Community Resources Knowledge: Medicaid Medicare Social Security HCBS Agency on Aging.

  • Special Populations: dementia psychiatric disorders substance use high-risk/vulnerable patients.
