Job Description

Job Summary:

This role is a community clinic-based case management position for a social worker responsible for conducting initial and 180-day assessments for participants coordinating hospital and skilled nursing facility discharges and collaborating with the interdisciplinary team. The position involves a moderate amount of home visits within a specified geographic area.
Location: Seattle Washington United States
Responsibilities:
- Conduct initial and 180-day assessments for assigned participants.
- Coordinate hospital discharges collaborating with hospital discharge planners to ensure safe and appropriate discharge plans.
- Coordinate skilled nursing facility (SNF) discharges.
- Collaborate with the interdisciplinary team (IDT) to ensure continuity of care.
- Conduct home visits to assess participants (moderate travel within a defined radius).
- Maintain accurate documentation.
- Travel between work locations as needed with mileage and travel time reimbursed.
Required Skills & Certifications:
- Masters degree in Social Work (MSW).
- One year of experience in a relevant field.
- Current license as a Licensed Clinical Social Worker (LICSW) Licensed Social Worker Associate in Independent Clinical Practice (LSWAIC) or Licensed Associate Social Worker (LASW) in Washington state.
- Basic Life Support (BLS) certification.
Preferred Skills & Certifications:
- Experience in case management or a related field.
- Experience working with geriatric populations or individuals with complex medical needs.
Special Considerations:
- Primarily outpatient setting.
- Moderate travel required within a specified geographic area (Edmonds to Auburn).
- Travel time and mileage are reimbursed. All travel is expected to occur within the workday (8 am - 5 pm).
- Personal commute is not reimbursed.
- Candidates have autonomy in scheduling home visits.
- Experience as a first-time traveler is not required; however first-time travelers are not accepted.
Scheduling:
- Five 8-hour days per week.
- Schedule is flexible with autonomy in scheduling home visits within the workday.
- No weekend or on-call requirements.
