Job Summary:
This role is a community-based case management position for a social worker within a PACE (Program of All-inclusive Care for the Elderly) setting. The social worker will conduct initial and 180-day assessments for participants coordinate hospital and skilled nursing facility discharges collaborate with the interdisciplinary team and conduct home visits within a specified geographic area. Mileage reimbursement is provided for travel between work locations during the workday.

Responsibilities:
- Conduct initial and 180-day assessments for PACE participants.
- Coordinate hospital discharges collaborating with discharge planners to ensure safe and appropriate plans.
- Coordinate skilled nursing facility discharges.
- Collaborate with the interdisciplinary team (IDT) to ensure continuity of care.
- Conduct home visits to assess participants (moderate to light travel).
- Maintain accurate documentation.
- Participate in IDT meetings.
Required Skills & Certifications:
- Masters degree in Social Work (MSW).
- One year of experience as a social worker.
- Current license as a Licensed Clinical Social Worker (LICSW) Licensed Social Worker Associate in Clinical Social Work (LSWAIC) or Licensed Associate Social Worker (LASW) in Washington state.
- Basic Life Support (BLS) certification.
Preferred Skills & Certifications:
- Experience in geriatric care or case management.
- Experience with PACE programs.
Special Considerations:
- Primarily outpatient setting.
- Moderate to light travel within a defined geographic area (Edmonds to Auburn).
- Mileage reimbursement for travel between work locations during the workday (8am-5pm).
- No weekend or on-call requirements.
- RTO policy to be confirmed with the hiring manager.
Scheduling:
- 5 eight-hour days per week.
- Flexible scheduling for home visits within the workday.
