We are seeking an experienced Masters-Level Social Worker (MSW LICSW/LSWAIC/LASW) to serve as a Case Manager within its community-based PACE program. This role focuses on case management participant assessments and discharge coordination.
Assignment Length: 8 weeks
Role: Case Manager (MSW)
Setting: Community clinic with moderate in-home visits (paid time mileage reimbursement)
First-Time Travelers: Not accepted must have prior travel experience
Locals: Accepted
Weekend / On-Call: No weekend requirement; no on-call duties
RTO: Must be listed at submission and confirmed with hiring manager
Conduct initial and 180-day biopsychosocial assessments for assigned participants.
Coordinate hospital discharges by working closely with hospital discharge planners to ensure safe and appropriate care transitions.
Manage SNF (Skilled Nursing Facility) discharges and collaborate with rehab facilities to coordinate continuity of care.
Serve as a core member of the Interdisciplinary Team (IDT) ensuring smooth communication and patient-centered planning.
Perform moderate in-home visits (within Edmonds Auburn radius) reimbursed mileage and paid travel time between participant locations.
Develop monitor and revise individualized plans of care including discharge and contingency planning.
Provide patient/caregiver education crisis intervention counseling and care coordination with community resources.
Document all activities interventions and assessments in compliance with Epic and Providence policies.
Minimum 2 years experience in Case Management or Social Work.
Travel experience required (first-time travelers not accepted).
Experience in community hospital settings; LTAC and Trauma Level I preferred.
Ability to manage caseload of up to 1:50 patient ratio.
Must be comfortable performing both clinic-based and in-home assessments.
Strong knowledge of community-based services discharge planning and social support systems.
Education:
Masters Degree in Social Work (MSW) required.
Experience:
At least 2 years of post-MSW experience in case management or social services.
Experience in hospital discharges SNF coordination and outpatient clinic settings.
License/Certifications:
Required: BLS MSW one active Washington State credential (WA LICSW WA LSWAIC or WA LASW).
Preferred: LMSW with additional clinical social work credentials.
Skills:
Case Management & Utilization Review (admissions continued stay authorizations appeals/denials).
Care coordination & interdisciplinary collaboration.
Knowledge of protective services (APS guardianship POA emergency detention).
Experience with cultural sensitivity vulnerable/high-risk populations and community resource navigation.
Biopsychosocial assessments dementia and psychiatric disorder screening.
Crisis intervention short-term therapy individual counseling and discharge education.
Familiarity with Medicare/Medicaid HCBS Medicare waiver and social security programs.
Charting/Documentation with Epic and SPM.
