This position is a community clinic base case management position. If hired you will be completing initial and 180 days assessments for participants on the panel you will be covering. You will be highly involved in hospital discharges assisting and communicating with the hospital discharge planner to ensure a safe and appropriate discharge plan while communicating the recommendations from the Providence PACE IDT. You will also coordinate SNF discharges as well if a participant discharged from the hospital or the community there for rehab or skilled needs. You will work within an IDT for continuity of care of the participants you oversee. A candidate in this position will do some though not all of their assessments in the homes of the participants. It is a moderate to light amount of in-field work and its definitely far less than a home health or hospice social worker would do. This could be within the radius of Edmonds to Auburn. A candidate will be reimbursed for mileage and hours driving from one work location to another. This will count as paid time worked (ex: MSW drives from PACE center to participants home for assessment then drives back to PACE center). It is expected that all travel will occur within the workday of 8am-5pm which would all be paid time. Mileage will be reimbursed when the candidate is traveling from one work location to another but the candidates commute from their personal home will not be paid. A candidate in this role will have a lot of autonomy in scheduling their home visits so visits will not be needed to be scheduled in connection with their personal commute.
