Job Description

Job Title: Medical Director - Geriatric Medicine

We are seeking a full-time Board-Certified Internist or Family Medicine physician with expertise in geriatrics for the Medical Director position at a community-based PACE program in beautiful Pensacola Florida. Our facility serves individuals aged 55 and older who require nursing home care yet can live safely in the community offering a unique approach to support independence and dignity for seniors. Join a dedicated team committed to providing a full continuum of care ensuring seniors receive all necessary medical and supportive services in their homes.

Job Responsibilities:

  • Conduct comprehensive assessments and develop care plans for new participants.
  • Evaluate applicants to the PACE program and contribute to Intake and Assessment decisions.
  • Discuss medical conditions and care planning with participants and their families.
  • Foster collaboration across interdisciplinary teams and supervise Primary Care Providers and Advanced Practice Providers.
  • Provide direction and support for nursing treatment plans.
  • Engage in team and family conferences to aid decision-making processes.
  • Educate participants and families on health maintenance and chronic conditions.
  • Perform physical examinations and interpret laboratory and diagnostic results.
  • Participate in provider on-call rotation as required.

Qualifications:

  • Current and active plenary physician license to practice in Florida.
  • Board Certification in Internal Medicine or Family Practice required; Geriatrics certification preferred.
  • Experience or added qualifications in geriatrics preferred.
  • Current CDS Certification and DEA Registration required.
  • Supervisory experience required.
  • Documented experience working with a frail or elderly population for a minimum of one year required.
  • Strong skills in physical assessment and chronic disease management for older adults required.
  • Desire to work in a creative and alternative healthcare setting required.
  • Able to manage changing priorities based on participant needs required.

License and Certification Requirements:

  • Active Florida physician license.
  • Board Certification in Internal Medicine or Family Practice.
  • CDS Certification and DEA Registration.
