Job Description
Duties:
- Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process
- Performs utilization management and quality screenings
Skills:
- 2 years of acute care experience strongly preferred
- Case Management Managed Care.
- In Patient Case Management is required
Education:
- BSN required
Schedule Notes:
- 8a -4p with weekend requirement
