Job Description

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this pivotal role you will be responsible for coordinating patient care ensuring compliance with medical protocols and facilitating effective communication among healthcare providers. Your expertise in patient hospice care and case management will be essential in delivering high-quality services to our patients particularly those requiring behavioral health support and discharge planning.

Responsibilities

  • Assess patient needs and develop individualized care plans based on medical history and current health status.
  • Coordinate with multidisciplinary teams to ensure comprehensive patient care across various settings including primary care and hospital medicine.
  • Utilize medical terminology effectively to communicate with healthcare professionals and document patient interactions accurately.
  • Implement discharge planning strategies to facilitate smooth transitions from hospital to home or other care facilities.
  • Ensure adherence to HIPAA regulations while managing sensitive patient information.
  • Provide education and support to patients and their families regarding treatment plans medications and follow-up care.
  • Monitor patient progress and adjust care plans as necessary particularly for those with complex needs related to behavioral health or trauma.


Requirements

  • A current Registered Nurse (RN) license is required in OK.
  • Previous experience in case management or a similar role is preferred.
  • Strong understanding of patient care protocols including discharge planning processes.
  • Knowledge of hospital medicine practices and the ability to navigate a Level I Trauma Center environment effectively.
  • Excellent communication skills both verbal and written with an emphasis on patient education.
