Description

The primary purpose of your job position is to assist the Director of Nursing in planning organizing developing and directing the day-to-day functions of the Nursing Department in accordance with current federal state and local standards guidelines and regulations that govern our facility and as may be directed by the Administrator Medical Director and/or the Director of Nursing to ensure that the highest degree of quality care is maintained at all times.

RESIDENT RIGHTS:

  • Maintain the confidentiality of all resident care information including protected health information. Report known or suspected incidents of unauthorized disclosure of such information.
  • Ensure that you treat all residents fairly and with kindness dignity and respect
  • Ensure that all nursing is provided in privacy
  • Knock before entering the residents room
  • Report all grievances and complaints made by the resident to the Nurse Supervisor/Licensed Practical Nurse
  • Report all allegations of resident abuse and/or misappropriation of residents property immediately to the Nurse Supervisor
  • Honor the residents refusal of treatments request. Report such requests to your supervisor
  • Communicate respectfully and appropriately respect culture differences among residents and staff

ADMINISTRATIVE FUNCTIONS:

  • Assist the Director of Nursing in planning developing organizing implementing evaluating and directing the day-to-day functions of the nursing department in accordance with current rules regulations and guidelines that govern the long-term care facility.
  • Participate in developing maintaining and updating our written policies and procedures that govern the day-to-day functions of the nursing department.
  • Ensure the reference material (i.e. PDRS regulations professional standards of practice etc.) maintained at the nurses stations is current. Recommend to the Director written material will assist the nursing department in meeting the day-to-day needs of residents.
  • Ensure that the Nursing Service Procedures Manual is always current and reflects the day-to-day nursing care procedures used by this facility.
  • Develop and periodically update nursing department objectives and statements of philosophy.
  • Assist in developing and implementing a nursing department organization structure.
  • Make written and oral reports/recommendations to the Director as necessary/required concerning the operation of the nursing department.
  • Periodically review the departments policies procedure manuals job descriptions etc. make recommendations for revisions of policies procedures etc. to the Director of Nursing.
  • Assist in developing methods for coordinating nursing with other resident services to ensure the continuity of the residents total regimen of care.
  • Participate in the development maintenance and implementation of the facilitys quality assurance program for the nursing department.
  • Participate in facility surveys (inspections) made by authorized government agencies as request by the Administrator or Director of Nursing.
  • Participate in developing implementing and periodically updating the residents written discharge plan.
  • Assist the resident and Social Worker in planning the nursing portion of the residents discharge plan.
  • Perform administrative duties such as completing medical forms reports evaluations studies charting etc. as necessary.
  • Monitor the facilitys QI QM and survey reports and provide the Director with recommendations that will be helpful in eliminating problem areas.
  • Participate in the development maintenance implementation and updating of the written policies and procedures for the administration storage and control of medication and supplies.
  • Agree not to disclose residents protected health information by not disclosing assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Serve on the Quality Assurance and Assessment Committee as directed.
  • Assist the Director in preparing written and/or reports for the nursing department programs and activities submit to such committee(s).
  • Schedule attend and participate in developing the agenda for the Care Plan Committee meetings as necessary.
  • Participate in regularly scheduled reviews of the discharge plan and make such available to the Care Plan Committee as required or as may be necessary.

PERSONAL FUNCTIONS:

  • Assist the Director of Nursing in the recruitment and selection of nursing department personnel.
  • Assist the Director in preparing performance evaluations as directed.
  • Make daily rounds of the nursing department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Report findings to Director.
  • Review complaints and grievances made or filed by department personnel. Make appropriate reports to the Director as required or as may be necessary.
  • Ensure that departmental disciplinary action is administered fairly and without regard to race color creed national origin age sex religion handicap or martial status.
  • Interpret the departments policies and procedures to personnel residents visitors and government agencies as necessary.
  • Report occupational exposures to blood body fluids infectious materials and hazardous chemicals in accordance with the facilitys policies and procedures governing accident and incidents.
  • Report known or suspected incidents of fraud to the Administrator.
  • Participate with the Educator in developing planning conducting and scheduling in-service training classes that provide instructions on how to do the job and ensure a well-educated nursing department.
  • Provide leadership training that includes the administrative and supervisory principles essential for Nurse Supervisors/Charge Nurses.
  • Assist the Director in planning and conducting universal precautions in-service trainings.
  • Assist the In-Service Educator in developing annual facility in-service training programs (i.e. OSHA TB HIPAA Abuse Prevention Safety Infection Control etc.)
  • Assist the In-Service Educator in developing advance directive in-service training programs for the staff and community.
  • Assist the In-Service Educator in the development of in-service training programs for the nursing staff. Ensure that these programs meet the continuing education requirements that allow licensed personnel. (e.g. RNS LPNS CNAS etc.) to keep their license on an active status.
  • Attend and participate in continuing education programs designed to keep abreast of changes in your profession as well as to maintain your license on current status.

NURSING CARE FUNCTIONS:

  • Participate in the interviewing and selection of residents for admission to the facility including the PASARR screenings.
  • Provide the Director with information relative to the nursing needs of the resident and to the nursing departments ability to meet those needs.
  • Inform nursing personnel of new admissions their expected time of arrival room assignment etc.
  • Make rounds with physician as necessary.
  • Encourage attending physician to record and sign progress notes physicians orders etc. on a timely basis and in accordance with current regulations.
  • Review nurses notes to ensure that they are informative and descriptive of the nursing care being provided that they reflect the residents response to the care and that such care is provided in accordance with the residents wishes.
  • Schedule daily rounds to observe residents and to determine if nursing needs are being met.
  • Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.
  • Report Medication Errors to the Director of Nursing.
  • Provide direct nursing care as necessary.
  • Ensure that residents who are unable to call for help are checked on frequently.

INFECTION PREVENTION:

  • Develop implement and evaluate the organizational infection prevention program.
  • Surveillance
  • Develop an annual surveillance plan based on the population(s) served services provided and analysis of surveillance data.
  • Utilize epidemiologic principles to conduct surveillance and investigations.
  • Evaluate and modify the surveillance plan as necessary
  • Develop interpret and assist with implementation of infection prevention and control policies and protocols.
  • Communicate infection prevention and control information and data to various committees and healthcare workers across the organization as assigned.
  • Create educational goals objectives and strategies using learning principles and available educational tools and technology.
  • Comply with regulatory and mandatory reporting requirements at the local state and federal levels.
  • Facilitate compliance with regulatory and accreditation standards.
  • Stay current on infection prevention and control regulatory and accreditation standards
  • Utilize PI methodology as a means of enacting change.
  • Define the scope of the project and select appropriate PI tools to aid in efficiency reliability effectiveness and ensure sustainability of the initiative.
  • Ensure that residents need and expectations are considered in the development of and continuous improvement of processes products and services.
  • Monitor and analyze process and outcome measures to evaluate the effectiveness and sustainability.
  • Participate in PI committees teams and initiatives as indicated.
  • Assess risk of occupational exposures to infectious disease.
  • Develop (or assist with) rates and trends of occupational exposures.
  • Develop (or assist with) immunization and screening programs.
  • Apply work restrictions and recommendations related to communicable diseases or following an exposure.
  • Consult and collaborate as needed with local state and federal public health officials and community health organizations.
  • Evaluate (critically) research and evidence-based practices and incorporate appropriate findings into routine practice.
  • Integrate evidence-based practices into policies guidelines protocols and educational strategies.
  • Identify barriers for implementation and develop strategies to minimize or remove barriers.
  • Implement strategies to sustain efforts such as audit tools and meaningful feedback.

SAFETY AND SANITATION:

  • Monitor your assigned personnel to ensure that they are following established safety regulations in the use of equipment and supplies.
  • Ensure that established departmental policies and procedures including dress codes are followed by your assigned nursing personnel.
  • Ensure that the adequate supply of personal protective equipment is on hand and are readily available to personnel who performs procedures that involve exposure to blood or body fluids.
  • Ensure that your units resident care rooms treatment areas etc. are maintained in a clean safe and sanitary manner.
  • Ensure that your assigned personnel follow established handwashing and hand hygiene technique in the administering of nursing care procedures.
  • Ensure that all personnel wear and/or use safety equipment and supplies (i.e. back brace mechanical lifts proper lifting technique etc.) when lifting or moving residents.
  • Have the knowledge of what is in the MSDS binders report missing/illegible labels and MSDSs to the safety officer or other designated person.

Qualifications

  • Must possess as a minimum a Nursing Degree from an accredited college or university.
  • Must have as a minimum two (2) years of experience as a supervisor in a hospital nursing care facility or other related health care.
  • Must have as a minimum of six (6) months experience in rehabilitative and restorative nursing practices.
  • Must possess a current unencumbered active license to practice as a Registered Nurse in this state.
  • Must be able to read write speak and understand the English language
  • Must be a supportive team member contribute to and be an example of teamwork and the team concept
  • Must possess the ability to deal tactfully with personnel residents family members visitors government agencies/personnel and the general public
  • Must be able to follow written and oral instructions
  • Must not pose a threat to the health or safety of their individuals in the workplace
  • Must be able to move intermittently throughout the workday
  • Must be able to function independently and have flexibility personal integrity and the ability to work effectively with other
  • Must be willing to perform tasks that may involve exposure to the blood and/or body fluids
  • Must be willing to assist in the evacuation of residents during emergency situations
  • Must be able to push pull move and/or lift a minimum of 25 pounds to a minimum height of 5 feet and be able to push pull move and/or carry such weight a minimum distance of 50 feet.
