1.0 FTE 40 hours per week

On-call required

Time will be split between liver transplant and living donor kidney transplant duties.

Ensures care continuity for patients and families and collaborates in the development implementation and evaluation of all program components.


Job Description:


Essential Functions:

  • Develops implements and enhances special projects promoting inpatient/outpatient continuity of care and parent engagement.
  • Ensures the provision of high quality individualized comprehensive cost effective patient across the continuum.
  • Initiates tracks analyzes and manages all program quality improvement and research projects program goals and objectives.
  • Coordinates functions with interdisciplinary medical care liaison functions with patient family medical staff and referring physicians.
  • Develops educational materials and programs for patients nursing and ancillary staff and physicians.
  • Develops clinical protocols policies and procedures including community education and public relations.

Education Requirement:

  • Bachelors Degree required.
  • Masters Degree in Nursing preferred.

Licensure Requirement:

Registered nurse in the State of Ohio required.


Certifications:

Skills:

  • Effective customer service skills.
  • Good multiplatform communication skills (oral written presentation).
  • Ability to assist in conflict resolution.
  • Problem-solving and interpersonal skills required for interaction with contacts required.

Experience:

Three years of registered nurse experience required.
