Job Description

Peaceful Pines Senior Living is committed to providing exceptional care and service with compassion. Our community offers independent living assisted living and memory care.

The Memory Care Coordinator is responsible for oversight of memory care staff including but not limited to scheduling training developing and coaching. The Memory Care Coordinator will also be responsible for creation and organization of holistic care for Memory Care residents. They will partner with the Director of Nursing to provide excellent care.

DUTIES AND RESPONSIBILITIES INCLUDE:

  • Collaborate with Director of Nursing (DON) and Memory Care Manager (MCM)/Assistant Director of Nursing (ADON) to create personalized holistic care plans and establish daily routines for each memory care resident.
  • Assisting with resident self-care including daily activities such as hygiene eating toileting activities medication administration and exercise.
  • Integrate dementia care with daily activities for residents that have significant cognitive impairment or deficits in motor functioning.
  • Provide support and improve memory care residents quality of life by reducing a patients confusion and supporting their individual needs with compassion and patience.
  • Research and advance dementia understanding and skills through trainings online videos etc.
  • Educate families and other care staff about a variety of dementia care therapies and memory-enhancing activities to best maintain the health and well-being of memory care patients.
  • Additional responsibilities as necessary to support the Memory Care residents needs.
  • Promotes and supports a positive work environment focused on team building and collaboration.
  • Maintains confidentiality of all pertinent personal or health information concerning residents and staff.
  • Ensures that all the needs are met for the residents as well as supervises staff that are on the shift.
  • In collaboration and guidance from DON/MCM/ADON:
    • Ensures that any incident reports are completed properly and promptly.
    • Prepares necessary medical paperwork for all scheduled resident appointments.
    • Assists with compliance in meeting regulations and is available for state licensure surveys.
    • Maintains resident files and appropriate documents as required.
    • Assists Medication Aides and Caregivers in service delivery and training as requested.
    • Notifies DON of any new doctors orders that are received and informs the DON of any med changes.
    • Schedules care conferences and updates care plans in accordance with regulations.
    • Observes residents records significant conditions and reactions and notifies DON/ADON/MCM family (responsible party) and physician of residents change in condition and reaction to drugs treatments and significant incidents.
    • Takes temperature pulse blood pressure and other vital signs to detect deviations from normal and informs DON/ADON/MCM the condition of residents.

PREFERRED QUALIFICATIONS:

  • Familiarity with PointClickCare software a plus.
  • Previous experience in a leadership role in an assisted living community.
  • Minimum of two years experience in a healthcare facility.
  • Strong interpersonal verbal and written communication skills.
  • Detail-oriented with the ability to plan and carry out job tasks independently.
  • Must be able to exercise discretion and solid judgment.
  • Ability to set priorities manage multiple tasks simultaneously and adapt efficiently to change.
  • Effectively communicate with all levels of employees and management.
