About the role and where you will be working:
The Case Coordinator supports and facilitates case coordination with GPs to increase efficiency in the delivery of quality primary end-of-life and palliative care (EOL&PC) and holistically improve resident outcomes and quality of life.
Please note this role is Fixed Term for 12 months
Well also be asking you to:-
To be considered for this role youll need:
Demonstrated experience delivering education training programs to clinical and non-clinical staff
Ability to support advanced care planning for residents through discussion with residents families and GPs.
Ability to coordinate case conferencing with GPs resident families and the home.
Ability to drive change and business improvements in collaboration with key stakeholders
