Adelaide Primary Health Network (Adelaide PHN) is an independent not for profit primary health care organisation based in Adelaide. The Role
The Contact and Capacity Building Coordinator Preventative Care will work closely with primary healthcare providers Residential Aged Care providers key partners and stakeholders across the Adelaide PHN region. The role will ensure engagement regarding immunisation and health screening and broader preventative care initiatives within and across the region is current and relevant to primary and aged care.

This position is being offered as full time and is funded until June 2026. Future employment is subject to ongoing funding.
