SUMMARY DESCRIPTION OF PROGRAM
Heartland Bridges houses two programs BridgesTransitional Living and Bridges Crisis Care. Transitional Living is a shortterm transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or cooccurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe secure environment for 35 days while stabilizing from a crisis before returning to the community where they live.
SUMMARY DESCRIPTION OF WORK
The Case Manager II serves as a direct care staff that leads intensive case management services for Bridges transitional living clients. This role coordinates supportive services designed to meet the needs of the client while working with them to develop a plan for permanent housing. Decision making problem solving and highly developed interpersonal skills are critical.
Compensation: between $17.95 and $21.40 per hour (wage is determined by total years of relevant experience)
Work Schedule: 40 hours per week 2:30 p.m. to 10:30 p.m. shifts schedule to be discussed upon interview

MINIMUM QUALIFICATIONS
- High School Diploma
- Two years of experience in behavioral or mental health services
- One year of experience working with adults with serious mental illness or substance use
- Valid drivers license and acceptable driving record
Essential Duties and Responsibilities
- Responds to crisis care referral calls assesses individual by gathering information and provides appropriate and accurate application verdict maintaining strict confidentiality.
- Completes enrollment/intake on eligible transitional and crisis clients.
- Documents client activity service plan progress and needs per established record keeping procedures.
- Provides problem solving and crisis intervention services to empower clients to develop selfsufficiency.
- Screens for mental health and substance use treatment needs and provide linkages to appropriate services.
- Assists clients in locating obtaining and retaining suitable housing though housing search and placement services.
- Develops supportive relationship with clients and responds to client needs during shift.
- Ensures required information is collected entered in prescribed formats and reported in a timely manner.
- Provides program information and referral services to clients.
- Monitors client selfadministration of medication and follows procedures of recording.
- Willingness and ability to obtain/maintain certification to administer nonparenteral medications in the state of Iowa.
- Accurately record client outcomes on designated databases
- Psychoeducates clients on life social and coping skills; meeting with clients for weekly Life Skills Inventory coaching sessions.
- Demonstrates knowledge of current community and HFS programs and resources to meet client needs including providing connections to mainstream benefits income support services and Integrated Health Home services.
- Utilizes traumainformed care person centered and culturally competent principles in providing services to clients.
- Evaluates efficiency and effectiveness of community service providers on an ongoing basis to ensure clients are receiving quality care.
- Orients clients to Bridges protocols and addresses client noncompliance as necessary to create a safe and secure environment for all residents and staff.
- Conducts client rooms safety checks and room searches per program protocol.
- Responsible for maintenance and upkeep of facility
- Able to work both independently with little supervision and as a part of a team.
- Always displays a courteous and caring attitude to the clientele volunteers and visitors of the Agency.
- Abides by all specific program and Agency procedures policies and requirements.
- Cooperates and collaborates with program area staff volunteers and other Agency staff.
- Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media.
- Creates maintains and shares as appropriate a dynamic selfcare plan.
- Essential functions of this job are to be performed on company physical work site.
- Performs other program related duties as assigned.
