As a fulltime case manager at the HamptonNewport News Community Services Board you will play a vital role in the Regional Reinvestment Project to optimize mental health care by reducing reliance on state psychiatric facilities. Your responsibilities will include monitoring inpatient treatment and crafting effective discharge plans that facilitate seamless transitions to community support systems. You will provide shortterm case management typically lasting less than ninety days ensuring that individuals receive the necessary resources and services for their ongoing recovery.

Candidates should possess a Bachelors degree in fields such as Social Work Psychology Sociology or Counseling paired with a minimum of one year of experience in case management services. Proficiency in relevant software and tools for case tracking and documentation is crucial for effectively managing client cases.
The ability to establish rapport with clients and navigate community resources is paramount for successful discharge planning and ongoing support. Additionally a valid Drivers License is required as transporting clients may be necessary to facilitate access to essential services.
