Department Overview


Childrens Ambulatory Care Coordination (ACC) Program utilizes a model that is centered on empowering patients and their families to achieve maximum level of functional health. The goal is to empower families to assume independent coordination and gain self sufficiency in managing their child and families needs. The intended outcomes of the program are to optimize the patient / family experience improve population health and reduce cost.


Childrens ACC staff partners with patients/families in the context of their health care home (HCH) / medical home to identify and provide support resources across the continuum of their childs services. The childs care coordination team will include the partnership of the child and their family primary care provider (PCP) individual health care home clinic coordinators and appropriate ACC staff. ACC staff can include APRN RN and service coordinators. This team will collaborate to support efficiency and accuracy in meeting the patient/family needs.

Coordinator support occurs through in person contact at clinic appointments in the care coordination office as well as telephonic and video visits.



Position Summary

In addition to the role as RN Ambulatory Care Coordinator; the supervisor will have leadership oversight of the Care Advocate Coordinators. The Supervisor is responsible for ensuring efficient daily departmental operations and oversee allocation of workforce resources and facilitate transitions of care and has a key role in problem resolution of identified issues. The Supervisor supports the Hospital Flow Case Management and Care Coordination Manager and program operations on all shifts. Supervision responsibilities include the RN Care Coordinator and Care Advocate Coordinators (CAC).

Other primary duties will also include:
* Enhancing cross department outreach and relationship building.
* Process improvement in partnership with the System Hospital Flow Case Management and Care Coordination Manager.
* Participate in projects that directly impact the Ambulatory Care Team within the team itself and across departments.
* Supervisor role will delegate participation and elicit feedback from team to foster staff engagement.
* Lead onboarding education for new staff and ensure that staff has knowledge of resources for success.
* Act as a team supervisor to support the needs of the clinics and hospital.
* Act as superuser for new or changing technologies and assistance with training staff.
* Assist manager in developing education and training resources for the team.
* This role will report directly to the System Hospital Flow Case Management and Car Coordination Manager

Education:
Required
* Bachelors degree in Nursing.
Preferred
* Advanced degree in nursing or related field

Required Experience:
* Minimum of 5 years in clinical role
* Minimum of 2 years working in care coordination role
* Informal leadership experience is preferred
* Direct multicultural clinical work with children and family
* Assessment and delivery of individualized multidisciplinary patient/family plan of care
* Community social psychological resource acquisition and navigation for patients and families
* Working knowledge of pediatric medical conditions including chronic comorbidities
* Ability to effectively utilize and document patient information and plan of care into Electronic Health Record and other computer applicationbased tools
* Demonstrated customer service conflict mitigation engagement and organizational skills
* Minimum of two years experience developing care management approaches for racially/ethnically/culturally/socioeconomically diverse patient populations

The posted salary represents a market competitive range based on salary survey benchmark data for similar roles in the local or national market. When determining individual pay rates we carefully consider a wide range of factors including but not limited to market indicators for the specific role the skills education training credentials and experience of the candidate internal equity and organizational needs.

In addition to your salary this position may be eligible for medical dental vision retirement and other fringe benefits. Positions that require night weekend or oncall work may be eligible for shift differentials or premium pay.
