One in five Medicare patients were readmitted to the hospital within thirty days of discharge in 2011 resulting in $17 billion in healthcare costs. Poorly coordinated care transitions increase the likelihood of a readmission and elderly patients are particularly vulnerable due to the uncertainty they experience as they move from one care team to the next. Improving care transitions has the potential to reduce readmissions by maximizing communication and coordination among providers and minimizing the uncertainty for patients and families. We are seeking a project coordinator to support the BWH Nursing Care Transitions Program a new innovative initiative to improve the coordination and continuity of care patients receive when discharged from BWH to Skilled Nursing Facilities (SNFs) and home. The program aims to optimize a patients health during this transition improve patient satisfaction and reduce readmissions.
The Project Coordinator will work closely with the BWPO Manager Care Transitions Nurse and Director of Ambulatory Nurse Care Coordination to support the development and management of daily operations. He or she will assist in the identification and resolution of patient issues that arise during care transitions support the communication and coordination of care between health care providers and collect and manage data to measure program outcomes. In addition he or she will interface with medical nursing quality and population health management professionals and assist in the collaboration of multiple hospital care transitions initiatives to promote a cohesive coordination of efforts.

QUALIFICATIONS:
Maintain a master list of patients in the Skilled Nursing facilities (SNF)
Track SNF patients and communication with SNFs in EHR as appropriate
For patients discharged from BWH to a Skilled Nursing Facility follow up with facilities to confirm team meetings discharge date and other pertinent information as directed by the Director of Ambulatory Care Coordination and Care Transitions Nurse
Maintains a collaborative working relationship with key stakeholders
Secure the discharge summary from the SNF scan and inbasket it to the patients primary care practice
Respond to questions/concerns and triage as needed
Support the workflow around 3 day waiver referrals to skilled nursing facilities (SNF) including tracking of CMS required reporting elements.
Prepare weekly and monthly program reports to track program progress and quality outcomes
Perform administrative duties and additional tasks as necessary to support the program
Performs other duties related to other BWHC Post Acute initiatives as needed.
SKILLS/ ABILITIES/ COMPETENCIES REQUIRED:
Bachelors degree required
High level of competence with MS Office Suite (particularly Word Excel and PowerPoint) and other software applications. Willingness to learn RedCap MS SharePoint and other technologies as necessary

Travel to various BWH/BWPO campus locations as well as to other PHS facilities
This is a grantfunded position through September 30 2016 with the possibility of extension thereafter based on available funding and job performance.
