SUMMARY
The Support Services Coordinator (SSC) performs varied and diverse work and is responsible for personcentered services to people served. The SSC has knowledge of and involvement with areas and programs related to Medicaid supports accurate documentation and billing processes for people served and ensures completion of all duties with due diligence conducive to Health Care Policy and Financing rules and regulations. This position ensures the safety of the people served and fellow employees in all operations.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Streamlines communications and coordination where multiple staff employees and clients need to work together.
Streamlines clear concise communications that are and will be promoted to all parties involved.
Perform multiple administrative functions:
Prepare monthly Residential documents for providers
Receive and verify monthly residential documents from providers
Date stamp and scan all documents
Upload scanned documents into the appropriate iTherapyDocs (iTD) account
File original scanned documents in Archive files
Retrieve all records and work files as needed by management
Ensured compliance with all quality assurance documentation requirements for people served in the residential program.
Ensure completion of the following Assessments and Plans for each Resident within 30 days of enrollment and then annually:
Level of Supervision Conducive to Individualized Service/Care Plan documentation
Natural Supports assessments are used to determine the unpaid support that is available to the person within the family or in the community.
Money Management assessments are used to determine a persons skill and ability to handle money safely in what amount and in what form.
Selfmedication assessments: are used to determine a persons skill and ability to selfadminister medications.
Rights and Responsibilities Assessments: are used to determine if a persons rights are restricted in any capacity what rights are prioritized to be addressed and what right or rights the person deems are most important.
Comprehensive Life Reviews: are used as an informational overview document to assist the Interdisciplinary Team in preparing other documentation for service and support/care plans. (Requires updating when a significant life change occurs (e.g. move to a new address; loss of a family member) or every two years.
Health and Safety Assessment & Plan: are used to determine a persons skills and abilities surrounding issues with health and safety.
Individual Residential Support Assessments: are used to determine and outline areas where support is needed.
Distribute collect review discuss with the Host Home Provider and file in Host Home binders annual Host Home Provider training.
Meet with Residents annually to complete Residential Host Home and other applicable services Satisfaction Survey(s).
Review all the residents files including but not limited to iTD accounts binders inventory medical forms etc. for compliance.
May participate in residential moves and home preparations as needed.
