Job Description

JOB DESCRIPTION

  • Position: ECM Case Manager
  • Pay Range: $24 $28
  • Reporting To: Director of ECM
  • Work Type: Onsite

POSITION SUMMARY

The ECM Clinical Case Manager is responsible for the clinical care coordination of ECM Program clients. Care coordination includes identifying organizing coordinating and monitoring services needed by a recipient. The ECM Case Manager assists ECM Program recipients in gaining access to services and other community resources. This position is mostly done in the field.

QUALIFICATIONS

  1. Minimum of twoyears experience working with homeless and/or low and mixedincome populations as well as substance abuse and severe mental health issues.
  2. Bachelors Degree in Health Care related field preferred.
  3. Knowledge of community and housing resources and government benefits/welfare system.
  4. The individual in this position must have good verbal and written communication skills as well the ability to make sound clinical judgments regarding client care.
  5. Must be a licensed driver with an automobile that is insured and is in good working order in accordance with state and/or organizational requirements.
  6. Possesses current CPR certification.
  7. Current and satisfactory report on preemployment physical examination including TB Screening Test or chest Xray as required by Agency policies and procedures. Must be free from signs of infection and illness.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  1. Responsible for coordinating with those individuals and/or entities to ensure a seamless experience for the Member and nonduplication of services.
  2. Engage eligible members.
  3. Complete a biopsychosocial assessment and a Care Plan. Oversees the implementation of the Care Plan.
  4. Develop effective and professional relationships with property owners/managers housing providers.
  5. Develop effective trusting relationships with clients with focus on facility their independence and longterm housing stability.
  6. Facilitate linkage to mental health substance abuse and medical services and provide transportation when needed.
  7. Communicate with mental health substance abuse and medical provider to ensure continuity of care.
  8. Connect Member to other social services and supports the Member may need including transportation.
  9. Advocate on behalf of Members with health care professionals.
  10. Use motivational interviewing trauma informed care and harmreduction approaches.
  11. Coordinate with hospital staff on discharge plan.
  12. Accompany member to office visits as needed and according to MCP guidelines.
  13. Monitor treatment adherence (including medication).
  14. Monitors service delivery adhering to the prescribed schedule of client contact. Conducts face to face visit with Members as needed.
  15. Maintains proper charting progress notes and case records for each enrolled Member.
  16. Track interventions and outcomes.
  17. Handles complaints from clients families or friends.
  18. Reports all signs of abuse or neglect
  19. Participates in Education Training and Quality Improvement Activities. Assists the Agency in maintaining compliance with Federal State Local and HIPAA Regulations or Joint Commission Standards.
  20. Establishes and maintains good relationships with all Health Plans and other vendors.
  21. Performs other duties that may be assigned.
