• Job Description: Dialysis Social Worker
• Pay Rate: $25.37 per hour
• Travel Package: Weekly Stipend of $1162.00 ($413.00 for weekly meals $749.00 for weekly lodging)
• Gross Weekly Pay: $2177
• Contract Duration: 14 weeks
• Shift: 8hour days
• Facility Location: Montgomery AL 36104

Job Responsibilities:

• Conduct initial assessments to understand the social emotional and financial needs of patients undergoing dialysis treatment.
• Provide counseling and emotional support to patients and their families to help them cope with the challenges of kidney failure and dialysis.
• Connect patients with community resources and support services such as financial assistance transportation and housing.
• Collaborate with healthcare teams to develop individualized care plans that address the holistic needs of patients.
• Advocate for patients rights and ensure they receive the best possible care.
• Provide education to patients and families about kidney disease dialysis treatment and selfcare practices.
• Organize and facilitate support groups for patients and families to share experiences and provide mutual support.

Requirements:

• Must have a Master of Social Work (MSW) degree.
• Must have a valid state license in Alabama.
• Previous experience in a dialysis setting is required.

Additional Information:

• Immediate start date.
• Float position: Will float between 3 clinics including 2 in Montgomery and 1 in Evergreen (75 miles south of Montgomery). Floats to Evergreen will likely occur 23 times per month as patient load requires.
• Guaranteed hours: 40 hours per week.
