Job Description
COVID VACCINATION (dose date on or after 10/3/23) NOW REQUIRED FOR THIS HEALTH SYSTEM.

RATE WILL BE ADJUSTED FOR LOCAL TRAVELERS (10% reduction of posted rate). PERM ADDRESS MUST BE UPLOADED WITH PROFILE.
DEFINITION/PRIMARY FUNCTIONS: Evaluates directs plans and provides treatments for patients with a wide variety of disabilities to help them achieve a maximum level of functional independence. Documents patient care and results of treatment appropriately in the medical record to comply with policies and procedures of the department and regulatory agencies.
POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
Requested time off during the course of this assignment MUST be noted and verified at upload. RTO REQUESTS AFTER UPLOAD WILL NOT BE APPROVED!!
NO MORE THAN 5 DAYS OF RTO WILL BE ACCEPTED (less is better).
8hrs nonbillable orientation
OT Holiday multipliers: 1.25%

Local Travelers (those who reside within 50 miles of facility) must be uploaded at a 10% reduction of posted rate/hour.
