Community Support Mentor
JOB HIGHLIGHTS
Responsibilities:
- Ability to support individuals to live independently in their community
- Work as a mentor and build relationships with individuals to assist them to connect to their community
- Ability to support individuals with daily activities needed to live independently. This can include but is not limited to cleaning cooking laundry medication support bill paying and personal care support
- Ability to develop community relationships that fosters connections between the person supported and community organizations (volunteer or join a club for example)
- Ability to make decisions and work independently
- This position requires a company email address which requires an application to be downloaded on your personal cell phone for multi-factor authentication (MFA) to ensure cyber security.
- Reliable vehicle (unless determined exempt with HR)
