We are seeking a deeply dedicated and highly organized Case Manager to serve as the central point of contact for individuals navigating a diverse spectrum of mental health services.
In this role you wont just manage filesyou will be fully invested in the lives and progress of our clients. You will serve as the vital bridge connecting clients and their families to essential programs includingABA HCBS CFTSS and OPWDD.
Key Responsibilities
Central Point of Contact: Serve as the primary point of contact between clients families care providers and state/local agencies to ensure seamless service delivery.
Cross-Service Coordination: Manage monitor and integrate care plans across multiple service models (ABA HCBS CFTSS and OPWDD).
Advocacy & Enrollment: Assist families with navigating complex eligibility requirements applications and re-certifications for OPWDD and Medicaid waiver programs.
Case Monitoring: Conduct regular check-ins assess ongoing client needs and update comprehensive care plans to reflect changing goals.
Documentation Excellence: Maintain precise compliant and up-to-date electronic health records in accordance with state regulations and organizational standards.
