Position Overview:

The Lead Nurse Practitioner at Psychogeriatric Services is pivotal in our partnerships with longterm care facilities. PGS contracts with these facilities to provide toptier behavioral healthcare services to their residents. As the Lead Nurse Practitioner you will act as a bridge between the clinical and administrative arms of PGS ensuring seamless operations and unparalleled patient care. This position is based in the eastern part of Pennsylvania primarily in and around Philadelphia. It is a hybrid remote position where some of your wok will be in facilities and some will be remote.

Key Responsibilities:

Clinical Duties:
  • Assess and diagnose patients formulating evidencebased treatment plans.
  • Prescribe medications and monitor their effectiveness.
  • Order and interpret lab tests ensuring timely and accurate patient feedback.
  • Provide patient and family education and offer therapy when necessary.
  • Ensure timely and accurate medical record documentation.
  • Refer patients for inpatient care or to specialized providers.
  • Be available for crisis consultation.
  • Collaborate with other specialists ensuring a holistic approach to patient care.
  • Exhibit expertise in the clinical assessment and treatment of patients.
  • Deliver multidisciplinary holistic care tailored to the unique needs of older patients especially those with comorbidities and frailty.
  • Floater coverage for facilities in the region

Training & Development:
  • Assist with onboarding new Nurse Practitioners (NPs) ensuring theyre wellequipped with the necessary tools and resources.
  • Manage shadowing and clinical orientations for new hires.
  • Offer coaching and consultations for NPs facing clinical challenges.
  • Act as a mentor and a resource for NPs
  • Offer clinical direction ensuring efficient workflow and staffing coverage.

Client Relationship & Coverage Management:
  • Provide consistent coverage and establish robust relationships at designated facilities.
  • Collaborate with facility staff to review patient census and ensure appropriate referrals.
  • Liaise with the administrative team to optimize the deployment and utilization of NPs in the region.

Requirements

Qualifications:

  • Master s Degree from an accredited Nurse Practitioner Program (Required).
  • Psychiatric/Mental Health Nurse Practitioner (strongly encouraged)
  • Two (2) years of related clinical experience as a PsychiatricMental Health Nurse PractitionerBoard Certified. (strongly encouraged)
  • Current Board Certification as a Nurse Practitioner (CRNP) by the state Board of Nursing.
  • Longterm care/nursing home experience (strongly encouraged)
  • Current American Heart Association Health Care Provider CPR certification is required.
  • Proven experience in a leadership role within a clinical setting.
  • Strong interpersonal and communication skills.
  • Ability to work remotely and effectively manage a team from a distance.
Physical Requirements:

Must be able to stand for up to 20 minutes at a time. Must be able to lift up to 15 pounds at times.
