This is a remote position.
We are looking for a proactive and compassionate individual to join our team as a Case Manager. In this role you will be responsible for providing comprehensive case management services to individuals in need of support.
Key Responsibilities:
Collaborate with assigned workgroups and project teams to fulfill case management duties.
Develop policies and procedures supporting the Intense Case Management Unit based on industry standards.
Create and maintain a detailed listing of community and state resources to aid individuals in accessing necessary support.
Work closely with other bureaus/contractors to address special needs and ensure continuity of service.
Perform the six essential activities of case management: Assessment Planning Implementation Coordination Monitoring and Evaluation.
Participate in Quality Improvement Programs aimed at enhancing processes and performance.
