Join our dynamic team at Wilkinson Staffing Agency (WSA) as a Mental Health/Substance Abuse Targeted Case Manager. In this role you will play a crucial part in coordinating and enhancing the wellbeing of NC Medicaid (Medicaid) or Health Choice (NCHC) beneficiaries facing serious emotional disturbances mental illnesses or substancerelated disorders.

Responsibilities:

1. **Case Management (MH/SA TCM):**

Perform individualized personcentered case management emphasizing empowerment and outcomefocused approaches.

Conduct comprehensive assessments integrating information from various sources to develop PersonCentered Plans (PCPs).

Collaborate with Community Care of North Carolina (CCNC) primary care physicians and obstetricians when applicable ensuring coordinated care.

2. **PersonCentered Planning:**

Facilitate personcentered planning to help beneficiaries achieve desired outcomes skills and symptom reduction.

Work in partnership with beneficiaries families and treatment providers to develop action plans that evolve with changing needs.

3. **Referral and Linkage:**

Connect beneficiaries with medical behavioral social and other support programs identified in the PCP.

Coordinate service delivery reduce fragmentation of care and facilitate access to necessary services.

4. **Monitoring and FollowUp:**

Ensure effective implementation of PCPs through ongoing monitoring and followup activities.

Evaluate the adequacy and effectiveness of services addressing changes in the beneficiarys needs or status.

Collaborate in developing a comprehensive PCP addressing all service and support needs.

Establish connections to natural supports minimizing crisis episodes.

Encourage independence in managing care promoting selfcare behaviors.
