PositionSummary:
ThisexcitingnewrolewillbesupportingtheCoordinatedAccessFacilitatorwiththemanagementof
theBy-NameList(BNL).Thisroleisdesignedtosupportthecriticalneedforindividualprioritization
forhousingandsupportaswellasaggregatedatatoinformresourceplanningandinvestmentinending
homelessness.
ABy-NameListisareal-timelistofallknownpeopleexperiencinghomelessnessinyourcommunity.
Itincludesarobustsetofdatapointsthatsupportcoordinatedaccessandprioritizationatahousehold
levelandanunderstandingofhomelessinflowandoutflowatasystemlevel.Thisreal-timeactionable
datasupporttriagetoservices,systemperformanceevaluationandadvocacy(forthepoliciesand
resourcesnecessarytoendhomelessness).
MajorResponsibilities:
TheBy-Name(BNL)PriorityListisupdatedregularlyandsharedwith the appropriatecommunitypartners.
ServeasthemaincustodianoftheBNL;ensuringnewclientsarebeingadded,existingclients.
arebeingupdatedandclientsarebeinghousedinthemostequitablemannerpossible.
Screeningintakesforaccuracy.
Regularmonitoringandfollow-upwithreferralsourcesonhousingreadinessrequirements.
EnterdatainanaccurateandconsistentmannerinaccordancewiththeBuiltForZero(BFZ)data
reportingprocess.
EngagewithcoordinatedaccessmemberstoverifyinformationontheBNL.
Sortandfilterspecificcohortsasrequested.
IdentifyingtargetpopulationsandtrendsontheBNL.
Providetrainingtocommunitypartnersandaccesspointstoensure the accuracyofstepsforBNL
intakeprocessasrequired.
AttendCoordinatedAccessmeetingsbi-weekly.
Accessing/ReferencingHomelessIndividualsandFamiliesInformationSystems(HIFIS).
SortingandmonitoringtheBNLbucketsandothertaskspertainingtoeffortsofourBuiltFor
Zeroinitiatives.
Assistingwiththecreationofprioritylistsforhousingvacancies.
Assistingwithdatarequestsandcreationofdatareportingsheets.
RespondingtocommunityrequestspertainingtotheBNL.
ConnectinginstitutionstooutreachforpurposeofconductingBNLintakes.
MonitoringVeteranstatusandpriorities.
AdditionalsystemsimplementationandimprovementworkasitpertainstotheBNL,
CoordinatedAccess(HousingAssessmentReviewTeam-HART)andBFZincollaboration
withtheCACoordinator(FormerlyknownastheAffordableHousingSpecialist)asrequired
Demonstratealevelofexpertiserelatedtothedutiesdescribednormallyacquiredthroughpost-
secondaryeducationinadisciplinepertinenttothisworksuchas;BusinessAdministration,Social
Work,CommunityPlanningor otherrelevant disciplines,includingcoursesinsocialservicedelivery,
business/partnershipdevelopmentandcommunityrelationsortheequivalentschool/work/experience
combination.
