Home Health Physical Therapist (PT) Travel Contract
Location: Medford Oregon
Position Overview
We are seeking an experienced Home Health Physical Therapist for a 13-week travel assignment serving Medford OR and surrounding communities. This position is ideal for therapists who are passionate about delivering individualized patient care in a home health setting while working independently within a supportive healthcare system.
Schedule
- Day Shift 5x8 Hours
- Hours: 8:00 AM 4:30 PM
- No weekends or on-call requirements
Responsibilities
- Provide skilled physical therapy services to patients in home health settings
- Evaluate patient conditions and develop individualized treatment plans
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and caregivers on therapy plans exercises and mobility techniques
- Complete accurate and timely clinical documentation using EPIC/Oasis systems
- Collaborate with interdisciplinary healthcare teams to coordinate patient care
- Manage a daily caseload of home health visits while maintaining productivity standards
Requirements
- Minimum 1 year of recent skilled Home Health Physical Therapy experience required
- Oasis experience required
- Active PT license required (pending license considered)
- BCLS certification required
- Must have reliable personal transportation for patient visits
- Strong communication and independent clinical decision-making skills
