Golden Club Coordinator (24415)

As a Golden Club Coordinator you will provide Day Programming for The Golden Club; a social club that aims to reduce isolation and increase social interaction with a variety of social physical and educational activities by means of a social model day program. You will report to the Supervisor of Prevocational and Senior Services and will work onsite.

Responsibilities:

• Develop and implement a recreation calendar to support socialization and independent living skills; plan for activities and identify supplies needed while adhering to budgetary guidelines identified.
• Coordinate and implement transportation plans to ensure participants have access to program activities.
• Engage in community-based activities of interest and relevance with participants in efforts to support increased and enduring community engagement of participants.
• Collaborate with collateral treatment team members regarding participants.
• Provide one-on-one guidance to participants with a focus on aligning interests strengths and capabilities with employment opportunities.
• Develop a placement or job-searching plan in accordance with the participants vocational interests and abilities.
• Complete case notes.
• Complete monthly reports.

Requirements and Education:

• A High School Diploma with at least 4 years of relevant experience; A Bachelors degree preferred.
• Experience with Day Programing and activity coordination for groups of 10-20 individuals with activities designed to enhance health and well-being by means of enhancing social interaction.
• Experience with individuals with mental health diagnoses and using behavior support techniques.
• Must have a valid NYS drivers license.
• Experience with Psychosocial Rehabilitation.
• Experience with the Adult population (50 and over).
• Experience accessing community resources on behalf of service participants.
