Job Title: Licensed Practical Nurse (LPN) Long-Term Care & Rehab
Location: Rochester NY 14612
Duration: 8-Week Contract
Schedule: 5x8-Hour Shifts (40 hours/week)
Shift: Primarily Evenings (Flexibility for Nights Required)
Job Summary
We are seeking an experienced Licensed Practical Nurse (LPN) to join a Long-Term Care and Rehabilitation setting. This role involves delivering high-quality patient care in a fast-paced team-oriented environment with flexibility to work across multiple units.
Key Responsibilities
- Provide direct patient care in long-term care (LTC) and rehabilitation settings
- Administer medications and perform prescribed treatments
- Monitor patient conditions and document accurately using Epic EMR
- Perform blood glucose monitoring and other routine clinical procedures
- Assist patients with activities of daily living (ADLs)
- Collaborate with interdisciplinary healthcare teams
- Float between units as required to support patient care needs
Required Qualifications
- Active New York Licensed Practical Nurse (LPN) License
- BLS (Basic Life Support) Certification
- Minimum 1 year of LPN experience in a long-term care setting
- Experience in Skilled Nursing Facility (SNF) or nursing home environment
- Experience with Epic EMR system
- Willingness to float across 3 units

- First-time travelers are welcome
- Weekend Requirement: Every other weekend (8 weekend shifts over 8 weeks)
- Local Candidates: Must reside at least 75 miles from the facility
- COVID-19 Vaccine: Not required
