Job Title: Registered Nurse (RN) Home Health Care

Job Description:
- Conduct patient assessments and develop individualized care plans
- Administer medications treatments and skilled nursing procedures
- Provide wound care IV therapy catheter care and patient education
- Monitor patient progress and communicate changes to physicians
- Coordinate care with therapists social workers aides and other healthcare professionals
- Supervise LPNs and Home Health Aides as required
- Complete accurate and timely documentation in compliance with Medicare CMS and agency standards
- Participate in case conferences and quality improvement activities
Requirement:
- Active RN license in good standing
- Minimum 1 year of clinical nursing experience (Home Health preferred)
- OASIS experience preferred
- Current BLS certification
- Valid drivers license and reliable transportation
- Strong assessment documentation and communication skills
Preferred Skills
- Home Health experience
- Case management experience
- Knowledge of Medicare regulations and OASIS documentation
