Job Summary
Responsible for assisting the needs of the Home Care Coordinator which include communication with Personal Care Contracted Companies and DME Contracted Companies. Assists in Setting up Personal Care Services in the home and ordering and tracking DME and supplies. Assists in the coordination of care between the families personal care companies and transportation. Performs telephone audits with the participants receiving personal care in the homes.
Responsibilities
- Communication
- Communicates with the Home Care Coordinators on a routine basis to assist in coordination of the care of the Participants.
- Communicates to the Personal Care Company the needs of the Participant for Personal Care. This includes changes in times and days of services for the Participant.
- Communicates to the Durable Medical Company the need for equipment and supplies.
- Sends all correspondence necessary to support the verbal communication by Email PACE Care EMR or FAX with appropriate information.
- Documentation
- Documents all Durable Medical Equipment in Appropriate Logs.
- Documents all Personal Care Services into Appropriate Categories in a template. Sends this document to appropriate team members. Understands importance of accurate data for On Call systems.
- Orders DME Supplies making sure that documentation is stored and connected properly.
- Documents in the EMR for the participant to update the Care Plan regarding Durable Medical Equipment and Personal Care Services.
- Quality
- Performs audits with Personal Care Services which includes phone calls to see that PCA has arrived at the time directed and to question any concerns that the participant may have and documents appropriately on Telephone Audit Log.
- Audits and records the actual day and time of delivery of equipment with every order entry.
Qualifications
Experience - One year working with the frail elderly.
Education - High School Diploma
Training - Current Certified Nursing Assistant Certification in the State of Louisiana
Special Skills - Computer Skills
BLS & First Aid Certification
