Job Summary
Registered Nurse (RN) Case Manager Hospice
Canonsburg PA & Surrounding Area
We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join our hospice care this role you will play a vital part in coordinating care and supporting patients and their families through every stage of the hospice journey.
Youll work alongside a collaborative and supportive team delivering high-quality patient-centered care while helping ensure comfort dignity and continuity of care.
Key Responsibilities
Manage and coordinate comprehensive hospice care for assigned patients
Collaborate with interdisciplinary healthcare teams to develop and implement care plans
Provide clinical leadership guidance and support to nursing staff
Monitor patient progress and adjust care plans as needed
Ensure accurate and timely documentation in compliance with care standards
Schedule & Work Expectations
Monday Friday 8:00 AM 5:00 PM
Productivity: 6 units per day / 30 units per week
On-call rotation:
1 weekend per month
1 evening per week
Territory
Primary coverage in Canonsburg PA
Travel within approximately a 1525 mile radius of the community
Local travel required across multiple counties
