Position Title: Travel Registered Nurse (RN) Case Manager
Location: Eureka CA 95501
Assignment Duration: 13 Weeks
Schedule: Night Shift 5x8 Hours (7:00 PM 7:00 AM)
Position Overview
Navitas Healthcare LLC is seeking a skilled and compassionate Registered Nurse (RN) Case Manager for a 13-week travel assignment in an acute care setting. This role is ideal for professionals with strong clinical expertise care coordination experience and a commitment to delivering high-quality patient outcomes.
Key Responsibilities
- Coordinate and oversee patient care plans across the continuum of care
- Collaborate with interdisciplinary teams to ensure effective treatment and discharge planning
- Conduct utilization reviews and monitor patient progress
- Adjust care plans based on clinical assessments and patient needs
- Ensure compliance with regulatory standards and documentation requirements
- Utilize electronic medical records (Epic) for charting and coordination
- Educate patients and families on care plans discharge instructions and available resources
Required Qualifications
- Active Registered Nurse (RN) license
- Minimum 2 years of acute care experience (required)
- Prior case management experience preferred
- First-time travelers considered (based on clinical evaluation)
- Strong communication organizational and critical thinking skills
