Job Description

Navitas Healthcare LLC isseeking an experienced Registered Nurse Case Manager for an acute care hospital setting. The ideal candidate will have recent hospital-based case management experience and strong knowledge of utilization review and care coordination processes.

Key Responsibilities

• Perform patient case management and care coordination in an acute care setting
• Utilize InterQual criteria for utilization review and discharge planning decisions
• Collaborate with physicians nurses and interdisciplinary teams
• Coordinate patient admissions transfers and discharge planning
• Ensure appropriate resource utilization and length-of-stay management
• Maintain accurate documentation in EPIC EMR
• Communicate with insurance providers and external care agencies as needed
• Support continuity of care across the patient care continuum

Required Qualifications

• Active Registered Nurse license in California (or pending license accepted)
• Recent acute care hospital case management experience (must be within last 6 months)
• Strong experience with InterQual and EPIC systems (hard requirement)
• Minimum recent experience in case management role
• Strong clinical judgment and utilization review skills
• Excellent communication and coordination abilities
