Registered Nurse (RN) Case Manager (Night Shift)
Location: Eureka California
Duration: 13 Weeks
Shift: Nights 5x8s (7:00 PM 7:00 AM)
Position Overview
Navitas Healthcare LLC is seeking a skilled and dedicated Registered Nurse Case Manager for a 13-week contract assignment in Eureka CA. This role is well-suited for an experienced acute care RN who excels in care coordination discharge planning and interdisciplinary collaboration within a fast-paced patient-focused environment.
Key Responsibilities
- Coordinate and manage patient care plans across the continuum of care
- Collaborate with physicians social workers and interdisciplinary care teams
- Perform utilization review and support effective discharge planning
- Ensure appropriate resource utilization and cost-effective patient care
- Document all case management activities accurately in EPIC EMR
Qualifications
- Active Registered Nurse (RN) license
- Minimum of 2 years of acute care nursing experience (required)
- Prior Case Management experience preferred
- Proficiency with EPIC EMR required
- First-time travelers accepted (subject to clinical approval)
