Position Overview
Navitas Healthcare LLC is seeking an experienced RN Case Manager to join a fast-paced acute care facility. The role focuses on care coordination utilization review and discharge planning while ensuring high-quality patient outcomes.
Key Responsibilities
- Coordinate care plans and discharge processes
- Perform utilization review using InterQual criteria
- Manage caseload (1:20 ratio)
- Collaborate with interdisciplinary teams
- Maintain accurate documentation in Epic EMR
Required Qualifications
- Active California RN License
- Recent acute care case management experience (within 6 months)
- Epic EMR experience (Required)
- InterQual knowledge (Required)
- First-time travelers not accepted
