Schedule

• Night Shift: 5x8-Hour Shifts
• Hours: 7:00 PM 7:00 AM
• Every other weekend required
• Float as needed based on department needs

Position Overview

Navitas Healthcare LLC is seeking an experienced Registered Nurse Case Manager with a strong acute care background for a contract opportunity in a fast-paced hospital setting. This role is responsible for coordinating patient care supporting discharge planning and ensuring effective communication across interdisciplinary teams to promote quality outcomes and continuity of care.

Key Responsibilities

• Manage patient care coordination utilization review discharge planning and transitions of care
• Review admissions denials/appeals benefits eligibility and prior authorizations
• Collaborate with physicians nursing staff social services and other healthcare professionals
• Maintain accurate and timely documentation using Epic EMR
• Support patient advocacy and ensure compliance with care management standards
• Float between units as needed to support operational requirements
• Manage caseloads of up to 25 patients per day

Required Qualifications

• Active RN license
• Minimum 2 years of recent acute hospital case management experience
• Strong background in care coordination and discharge planning
• Experience with HMOs IPAs advance directives POLST and protective services processes
• Current acute care experience within the last year required
• Epic charting experience required
• Previous travel experience preferred
• Strong communication organizational and multitasking skills

• Strong interdisciplinary collaboration and patient advocacy skills
• Flexibility to adapt to changing patient care needs and unit assignments
