Overview

JOB SUMMARY:

The Discharge Planner provides services to patients families and others in all hospital areas specific to assignment as applicable and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include but are not limited to case consultation patient education resource management and assistance with discharge planning.

EDUCATION EXPERIENCE TRAINING

High School Diploma and at least 2 years of acute care experience.

. BCLS Card within 60 days of hire and maintain.
