Registered Nurse (RN) Home Health Fort Wayne IN

You became a nurse to care for people not rush through a shift.

At Elara Caring you provide one-on-one care in the patients home. You manage your day build real relationships and see the impact of your work in real time.

What youll do:
Manage your own patient caseload
Complete start of care and follow-up visits
Develop and update care plans
Communicate with physicians and care team
Document visits accurately (HomeCare HomeBase)
Monitor patient progress and adjust care as needed

What you need:
Active RN license (Indiana)
1 year clinical nursing experience
Valid drivers license and reliable vehicle
Ability to travel within Fort Wayne and surrounding areas
Home Health or Hospice experience preferred
