Care Coordination RN Guide Patients Through Their Mental Health Recovery Journey

Position: Care Coordination RN
Location: Hays Kansas
Pay: starting at $57061 (based on experience) $500 SIGN-ON BONUS!
Job Type: Full-Time In-Person
Schedule: typical hours Monday-Friday 8am-5pm

At High Plains Mental Health Center our Care Coordination RN will play a critical role in helping individuals successfully navigate these transitions. Youll work alongside hospitals psychiatric facilities providers and community partners to ensure patients receive safe continuous and well-coordinated care.

What Youll Do:
- Help patients successfully transition from inpatient psychiatric units hospitals and residential programs back into community care
- Work closely with providers nurses and community partners to keep treatment plans aligned and effective
- Improve patient safety by reviewing medications and coordinating follow-up care
- Educate and support patients so they understand their care plan and feel confident moving forward
- Build relationships across healthcare systems that strengthen mental health care in our region

- Current Registered Nurse (RN) licensure from the Kansas State Board of Nursing. Preferred- Bachelor of Science in Nursing (BSN).
- Preferred: experience in psychiatric/mental health nursing or physician office or outpatient practice.
- Preferred: experience in care coordination or case management.
- Strong interpersonal and collaboration skills when working with multidisciplinary teams and external providers.
- Ability to communicate effectively with patients and families about treatment plans and care transitions.
- Demonstrated proficiency with technology including computers electronic records and common office equipment.
- Commitment to continuous learning and quality improvement particularly in mental health care and clinical processes.
