Choices is seeking an Independent Contractor who will market and develop referrals for behavioral health services for children and adolescents across South Mississippi. Referrals may originate from schools pediatric and family medicine providers hospitals child welfare agencies juvenile justice entities and other community organizations serving youth. Referrals will be submitted directly to the Independent Contractor by community partners or identified through outreach activities and will be forwarded to the Agency Intake Coordinator within 24 hours of receipt.
Essential Duties and Responsibilities
- Responsible for conducting outreach and relationship development activities with referral sources including meetings presentations and communications to educate partners on available pediatric behavioral health services eligibility criteria and referral procedures.
- Responsible for receiving referrals gathering required demographic and referral information and coordinating intake scheduling with Agency clinical staff.
- Maintain a referral tracking system documenting referral source referral date youth demographics scheduling status and intake completion status.
- Follow up with referral sources and families to support completion of scheduled intake appointments and reduce missed referrals.
- Forward referral information and scheduling updates to the Executive Director or designee as requested.
- Submit a monthly signed activity summary and invoice to the Agency Executive Director documenting outreach activities referral sources contacted referrals received intakes scheduled and completed intakes for that month.
- Notify the Agency Executive Director via email of any conflicts concerns schedule changes or needs that may arise related to referral development and coordination activities.
- The quality and effectiveness of referral development activities will be monitored by the Executive Director. Feedback from referral partners may be solicited on an ongoing basis.
- May participate in Agency meetings or planning initiatives related to referral development and community outreach as requested by the Executive Director.
Qualifications:
High school diploma or equivalent required; associate or bachelors degree in social work psychology public health healthcare administration communications or a related field preferred.
At least 12 years of experience in community outreach marketing referral coordination healthcare behavioral health or social services.
Strong communication and relationship-building skills with the ability to engage community partners and referral sources.
Ability to collect organize and document referral information accurately and maintain a referral tracking system.
Ability to coordinate scheduling and follow up with families and referral sources to support intake completion.
Strong organizational and time management skills and ability to work independently.
Basic computer proficiency including email spreadsheets and documentation systems.
Ability to maintain confidentiality and handle sensitive information appropriately.
Reliable transportation and ability to travel within South Mississippi for outreach activities.
Compensation: $20 an hour $50 per intake until exceed 20 intakes in the month and then $75 an intake
