Position Summary
The Licensed Master Social Worker (LMSW) provides social work services to patients in a hospital setting. The role involves supporting patients and families by addressing psychosocial needs assisting with care coordination and collaborating with interdisciplinary healthcare teams to improve patient outcomes.
Key Responsibilities
Provide social work services for patients in inpatient units including medicine surgery and rehabilitation (med/surg).
Conduct patient assessments to identify psychosocial emotional and support needs.
Collaborate with physicians nurses and other healthcare professionals to coordinate patient care.
Assist patients and families with discharge planning and transition of care.
Provide counseling crisis intervention and resource referrals when needed.
Maintain accurate documentation in accordance with facility policies and regulatory requirements.
Monitor patient conditions including basic patient observations such as vital signs when required.
Participate in interdisciplinary meetings and care planning discussions.
Required Qualifications
Education
Masters Degree in Social Work (MSW) from an accredited institution.
Licensure
Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED).
Experience
Minimum 1 year of social work experience.
Hospital or inpatient care experience preferred particularly in medicine surgery or rehabilitation settings.
Certifications
Basic Life Support (BLS) certification required.
Required Skills
Knowledge of hospital-based social work practices
Ability to assess patient psychosocial needs
Strong communication and interdisciplinary collaboration skills
Familiarity with patient discharge planning and care coordination
Ability to work in a fast-paced healthcare environment
