Job Description:
- Completes bio-psychosocial assessments suicide risk assessments and other clinical forms as required under the supervision of an independently licensed mental health professional.
- Provides individual and group psychoeducation and crisis intervention services under the supervision of an independently licensed mental health professional.
- Coordinates with the multi-disciplinary team regarding patient care.
- Develops and coordinates patient discharge/re-entry plans.
- Obtains relevant treatment records and coordinates care with family members community providers and others relevant to the patients care and community re-entry plans.
- Maintains and updates a list of community resources to support the patients transition and re-entry back to the community including civil commitment procedures Regional Behavioral Health Authority (RBHA) services and criminal justice processes.
Requirements
Minimum Qualifications:
- Diploma/transcripts to support graduation with a bachelors degree in behavioral health social services or a related field.
- Current valid unrestricted license as a Licensed Master Social Worker (LMSW) or Licensed Associate Counselor (LAC) or Licensed Associate Marriage and Family Therapist (LAMFT) issued by the Arizona State Board of Behavioral Health Examiners.
- Current completed PSV from the Arizona State Board of Behavioral Health Examiners.
- Signed attestation of current status as enroute to independent licensures within three years of completed orientation.
- Minimum one year of past work experience in this position.
Required Skills:
Conduct patient evaluations histories and physical exams for new and follow-up cardiology patients Manage and monitor patients with cardiovascular conditions such as hypertension coronary artery disease atrial fibrillation heart failure and hyperlipidemia Order and interpret diagnostic tests including EKGs Holter monitors echocardiograms and stress tests Assist in care coordination for procedures or specialty referrals Provide inpatient cardiology consults and follow-ups (if applicable) Participate in hospital rounds discharge planning and transitions of care Educate patients and families on lifestyle changes medications and disease management Collaborate closely with supervising cardiologists primary care providers and support staff
Required Education:
Current and unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA) in Hawaii or eligibility to obtain one National certification (AANP ANCC or NCCPA) BLS and ACLS certification required 12 years cardiology or internal medicine experience preferred; strong new graduates with relevant rotations or RN cardiology background considered Excellent interpersonal and communication skills Comfortable working independently with physician support EMR experience
