Your role and responsibilities
Our Transitions Team is part of a dynamic and evolving service that puts people first. We work creatively and collaboratively to support young people progressing to adulthood to live fulfilling independent lives.
As Team Manager you will:
- Lead and support a dedicated team of social care professionals.
- Promote a culture of continuous learning and development.
- Ensure statutory duties are met under the Care Act 2014 Mental Capacity Act 2005 and Mental Health Act 1983 (2007).
- Work closely with partners across childrens services health advocacy and community services.
- Enhance team practice to deliver collaborative responses providing the right information advice and enablement to young people to support them in achieving their outcomes in life.
You will need
- A relevant professional qualification and/or current and valid registration with Social Work England.
- Proven experience in Mental Health Learning Disability and Autism services and multiagency collaboration including the ability to interface between children adult educational health and community services.
- Strong leadership skills and the ability to guide teams through change.
- A commitment to risk-positive strengths-based practice.
- An interest in innovative approaches including digital solutions.

The team
Our Transitions team supports young people with functional mental health conditions learning disabilities and/or autism as they move into adulthood. Using a strengthsbased approach and the Signs of Safety and Wellbeing model we help individuals build confidence independence and achieve their goals.
You will join a friendly multidisciplinary team based at Constantine and Endeavour House in Ipswich working countywide within a flexible hybrid model. As part of the Mental Health Learning Disability and Autism Service youll contribute to strengthsbased assessments identify young people who may need support and work closely with carers and support networks.
We work primarily under the Care Act 2014 and Mental Capacity Act 2005 with awareness of relevant Childrens and Adult legislation. The service has a strong supportive culture and a commitment to continuous professional development.
