Behavioral Health Care Manager

About the Role:

The Behavioral Health Care Manager plays a critical role in coordinating and managing comprehensive behavioral health services to improve patient outcomes and quality of life. This position involves collaborating closely with healthcare providers patients and community resources to develop and implement individualized care plans that address mental health and substance use disorders. The manager will oversee patient progress facilitate communication among multidisciplinary teams and ensure adherence to evidence-based practices and regulatory requirements. Minimum Qualifications:

• Bachelors degree in nursing social work psychology or a related behavioral health field.
• At least 3 years of experience in behavioral health care management or a similar clinical coordination role.
• Strong knowledge of mental health disorders substance use treatment modalities and community resources.
• Familiarity with healthcare regulations privacy laws (such as HIPAA) and documentation standards.
• Excellent communication and interpersonal skills to effectively engage with patients and multidisciplinary teams.

Preferred Qualifications:

• Masters degree in social work counseling nursing or public health.
• Certification in case management or behavioral health care coordination (e.g. CCM CPHQ).
• Experience working within integrated care settings or accountable care organizations.
• Proficiency with electronic health record (EHR) systems and data analytics tools.
• Bilingual abilities or experience serving diverse cultural populations.

Responsibilities:

• Conduct comprehensive assessments of patients behavioral health needs and develop personalized care plans.
• Coordinate and monitor patient care activities including referrals to specialists therapy sessions and community support services.
