Requirements
As a Vice President of Care Management you will:
- Own the management and leadership across all care management operations. Full oversight of care management operations including:
- Staffing model and deployment of resources
- Personnel decisions
- Structure of team and authority over restructuring if needed
- Care management program offering and optimization (in partnership with Navvis enterprise)
- Collaborate with Navvis clinical ops team to evolve programs and ensure best practice/industry standards
- Evolve and lead care management workflow adherence and priorities
- Track and monitor performance and productivity standards and goals
- Train and coach care management staff regardless of badging
Lead a team of Navvis and SSM-badged colleagues responsible for performance outcomes in Ambulatory Care Management and Quality Operations. Notable Navvis and SSM badged reports include:
- Navvis VP Client Operations
- Navvis Director Quality Operations
- SSM badged Director Clinical Operations; 2 FTEs (St Louis and Madison)
- Establish and maintain a feedback loop with SSM leadership on care management leading and lagging indicators demonstrating tangible progress and achievement to established goals
- Foster positive relationships and excitement for the vision and journey to healthcare transformation including innovative and next level performance opportunities
- Responsible for staffing models change management and performance of care management and quality operations
- Develop achievable goals with direct reports setting the vision and standard for cascading those goals within the Care Management and Quality Operations teams
- Partner cross-functionally to successfully develop strategies to further impact value-based performance including implementation planning and performance goals
- Be accountable for performance of productivity and performance metrics of care teams
- Partner with Navvis analytics and reporting teams to optimize dashboards and reporting for leaderships and care management staff
- Partner with Navvis colleagues and departments to monitor performance indicators against identified outcomes and utilization goals
- Participate in partner governance forums representing scope performance best practice improvements and outcomes of care management
- Actively sponsor the implementation of organizational strategies and initiatives relative to Navvis services
- Engage and drive strategic initiatives from senior leadership
- Lead and provide oversight to direct reports and operations teams for:
- Establish workflows and policy and procedures needed for day-to-day operations
- Identify and scope operational improvements for care management impact including but not limited to technology advancements AI workflow and engagement optimization patient identification optimization and automation
- Collaborate with the clinical leadership to identify quality outcomes initiatives that connect to and elevate current and future care management impacts
- Continuously track and evolve population stratification criteria
- Ensure patients experience of care is reflected by a very high level of satisfaction by establishing patient survey processes and capabilities
- Integrate care management throughout other VBC functions including post-acute practice optimization physician enterprise payer strategy etc.
- Will perform duties and responsibilities in a manner consistent with our mission values and Service Standards
What Success Looks Like in this Role:
- The VP of Care Management owns and drives the operational success of care management programs
- Leads collaborative efforts with healthcare partners on strategic and operational initiatives and provides guidance to shared services functional areas supporting our financial model
- The VP of Care Management works regularly with the Navvis leadership team physicians and client partners to build relationships monitor performance improve processes solve problems and capitalize on opportunities to strengthen the company
- This position ensures operative objectives and results are in accord with Navvis overall needs and standards
- This role reports directly to the Market President with strong alignment also to the Navvis enterprise
Requirements
We are excited about you if you have these things:
- Education: A Bachelors or Masters Degree in a clinical field
- Extensive experience within the ambulatory operations space
- Experience: Minimum of 10 years of healthcare leadership experience directly leading a sizeable Care Management operation that includes ownership of performance outcomes quality measures and total cost of care value-based care contracts
- Expert knowledge of health care management principles and industry best practices
- Possesses and maintains a current knowledge of healthcare industry operations and trends
- Excellent communication skills with leadership outside partners patients and family.
- Ability to relate to physicians nurses and senior level health care leaders
- Excellent presentation skills
- Excellent interpersonal and patient centric skills to interact effectively with co-workers and outside partners
- Must be a proactive team-focused leader and self-starter
- Ability to understand strategy and how to implement strategy
- Strong and proven ability to develop coach and mentor team members
- Strong collaborative negotiating influencing leadership and decision-making skills
- Proven track record working with clients; excellent client interface and customer service skills
- Ability to manage change in a complex organization
- Strong interpersonal skills ability to work well in a multi-cultural environment and matrix organization
- High level of integrity with a hands-on work ethic and approach
- Superior leadership skills with the ability to influence and engage others direct and non-direct reports and peers
- Ability to travel into the St Louis and Madison markets on a frequent basis to meet with the team face to face to enable alignment with goals and expectations. Expectations of this role are 50% onsite with the care management teams in St. Louis and Madison Wisconsin.
